Core Requirements Form

Below you will find a list of our required trainings. We recommend that you check off each training as it is completed and attach a copy of your certificate to the back. After completion of each course, please mail a copy of your certificate to the address on the bottom of the form. Upon completion of the required courses, please print this form, sign and date it, and return it to us to receive your volunteer “Jump Kit.”

Orientation

• This is a prerequisite course to ALL other courses. This is only held in a classroom setting. Please check our Calendar for scheduling information.

Basic First Aid

• This is only held in a classroom setting. Please check our Calendar for scheduling information.
• Note: This course is offered along with BLS CPR and AED training or separately for those already maintaining a certification in CPR and AED. This course is not required for healthcare professionals that maintain a current certification.

BLS CPR/AED or BLS for the Healthcare Professional

• This is only held in a classroom setting.Please check our Calendar for scheduling information.
• You may find the text book on Amazon.com. Be sure to purchase the 2006 or higher version!

Incident Command System (ICS) 100

• This course is held in a classroom setting.Please check our Calendar for scheduling information.

National Incident Management System (NIMS) 700

• This course is held in a classroom setting.Please check our Calendar for scheduling information.
